Enrollment PLEASE READ THIS PAGE CARFULLY BEFORE PRINTING OUT THE ENROLLMENT FORM BELOW Classes meet for 30 to 40 minutes each week
depending on the size and level of the class. Adults must have a six string acoustic guitar. Children & pre-teens through age 13 must have a nylon string classical guitar. All guitars must be the appropriate size
and type. To enroll at Miller Guitar Academy: 1. Click on the button below and print out the enrollment/agreement form and fill out the information requested on the form. By submitting this form with a deposit you are agreeing to commit to the semester indicated and must pay for all fees upon billing. 2. Make sure you cross out the
times that you are unavailable for lessons
for reasons such as work or school. Once classes are set at a certain
time each week it cannot be changed. 3. Mail in the form with a deposit of $50 in a check made payable to Miller Guitar Academy. The deposit is non-refundable unless we cannot place you. Click here to print out the form. Once we receive your form and deposit you are officially enrolled and will be billed for the remaining tuition balance and materials fees before orientation. 4. You will be contacted by us to confirm your enrollment and make an appointment for an initial new student meeting. At this meeting you can meet your instructor and ask any questions you may have. If you need a guitar we will show you what is available for rent or purchase. If you have a guitar, we will look at it to make sure it meets our minimum requirements for beginning our program.
